Child Care COVID-19 Screening Tool for Workers

NOTE: This tool is for staff and workers only. If you are a Parent/Guardian trying to bring your child to a child care program, please use the COVID Pre-Screening Tool for attendees.

Please complete all questions and submit your results. Once submitted, an email will be sent to the email address provided in the tool below. This email must be shown prior to entering the child care program. If you do not show the email, you will be asked to fill in the form again and show the email at that time. There will be NO EXCEPTIONS. Thank you for your co-operation.

Are you currently experiencing any of these symptoms?

  • Fever and/or chills (feeling hot to the touch, a temperature of 37.8 C/100 F or higher)
  • Cough or barking cough (croup)
  • Shortness of breath (out of breath, unable to breathe deeply)
  • Sore throat
  • Difficulty swallowing
  • Runny and/or stuffy, congested nose (not related to seasonal allergies or other known causes or conditions)
  • Pink eye (conjunctivitis)
  • Headache that’s unusual or long lasting
  • Digestive issues (nausea/vomiting, diarrhea, stomach pain)
  • Muscle aches that are unusual or long lasting
  • Extreme tiredness that is unusual (fatigue, lack of energy)
  • Falling down often

Does a member of your household have any of the following new or worsening symptoms?

Symptoms should not be chronic or related to other known causes or conditions.
